We have provided the following forms
if you want to review them or
if you want to complete your first appointment paperwork
prior to your arrival for your first session.

If you have not completed your first appointment
paperwork prior to your arrival,
we will leave the forms for you on a clipboard
on a table in the waiting room. 
Please arrive 20 minutes before your scheduled
appointment time to complete them.   
 

For adults age 18 and older
Please complete the following two forms
and bring them to your first appointment. 
Each person who will be attending the first session
should complete a Client Information Form
and sign a Psychotherapy Agreement
(two people can sign one copy
of the Psychotherapy Agreement).

For therapy with minors under the age of 18
Please complete the following three forms
and bring them to the first appointment. 
Each person who will be attending the first session
should complete a Client Information Form. 
One or two parents or legal guardians should sign
a Parent's Consent Form and
a Psychotherapy Agreement for Minors. 
The minor should also sign the
Psychotherapy Agreement for Minors
(the minor and parent(s) can sign one copy
of the Psychotherapy Agreement for Minors).
